1. Review ATHLETIC ONLINE REGISTRATION INSTRUCTIONS
2. Complete ONLINE REGISTRATION
3. Submit completed SPORTS PARTICIPATION HEALTH RECORD FORM signed by a medical doctor (MD or DO ONLY). The form must be completed on May 1, 2025 or after and submitted to the Athletic Office prior to try-outs.  DO NOT upload your Sports Participation Health Record. Please submit the original form to the Athletics Office.
4. Verify all fines are cleared through the Finance Office - Don't wait.
5. Incoming Freshman should use 0000 for ORHS student ID # until they receive their permanent #.

• Submit completed MEDICATION IN SCHOOL FORM if student athlete is required to take medication during school activities
• Submit completed ASTHMA FORM if student has asthma
